Do you have each of these? You will need to provide copies if you are hired. Tell us "In progress" or "No" and we will help you get them.

Level One Fingerprint Clearance Card (front and back copy)
Proof of education (diploma, GED, or transcripts)
Current resume (proof of work experience)
Employment references from the last 2 years, plus 3 reference letters (at least one not related to you)
Copy of Social Security card
Valid Arizona driver's license
Proof of insurance on your personal vehicle
CPR and First Aid cards (adult and pediatric, taken in person)
CPI (Blue Card) or Article 9 training certification
R.P.P.S. (Reasonable and Prudent Parent Standard) certification
Sex Trafficking Awareness class certificate
Initial Health Care training (24 hour)
Food Handler's card
Medical exam showing you are free of communicable diseases
Proof of eligibility to work in the U.S.
Have you lived in another state in the past 5 years?

At hire you will also complete a notarized criminal-offense self-disclosure, a Central Registry background check (and an Adam Walsh check if you answered Yes above), a pre-employment drug test, and orientation.
